We understand that you have many choices when it comes to your medical care.  We thank you for choosing us.  This page was created to help our new patients become familiarized with our office policies.  If you have additional questions after reviewing this page, we encourage you to contact us.  We'll be happy to answer any questions that you may have.  We look forward to seeing you in the office soon!

Appointments

We see patients by appointment only. 

If you are unable to keep a scheduled appointment, we ask that you please give us 24 hours notice so that we may give your appointment to someone else in need.

Prescription Refills

For prescription refill, please call your pharmacy and have them fax such request to us at 940-552-2530. Please allow 48 hours to process prescription refill requests. As a general policy, we do not request refills after hours or on weekends.

Financial / Insurance

We accept most major health insurance plans.  To confirm that we accept your insurance plan, we encourage you to contact our office to confirm.

At the time of your visit, please be prepared to pay any co-payments, deductibles or non-covered services.  For your convenience, we accept cash, checks and debit/credit cards.

Patient Registration

For the convenience of our patients, we offer online registration through our secure patient portal.  Using the online portal to register helps to expedite the registration process.

For your first appointment, please also bring a government issued photo ID and your insurance card if you have insurance.  Please also remember to bring a list of any medications that you are currently taking, including any over the counter medications.
